The facility's business plan was not submitted electronically to the California Environmental Reporting System (CERS)for 2019 and 2020. As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class 11. 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2019 and 2020. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class 11. 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. One 500 gallon diesel tank, three 250 cubic feet compressed oxygen cylinders, three cylinders (at least 300 cubic feet, actual amount was not available as volume label was not visible on one cylinder), and the seasonal amount of propane of 176 gallons of propane have not been reported for 2019 and 2020. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. This is a repeat violation, Class 11. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. A site map was not completed or submitted as part of the business plan for 2019 and 2020. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA".
