If <br /> you cannot obtain a consolidated manifest (similar to the other manifest receipts you have on file for other pick-ups), <br /> you have the option of correcting this violation by submitting a statement of action you will take in the future with <br /> regards to ensuring that the manifest signed by you, the generator, contains all required information. <br /> 1) The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative. <br /> 2) The date of the shipment; <br /> 3) The manifest number; <br /> 4) The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste <br /> stream type listed in subdivision (c), and its proper shipping description, including the hazardous class and <br /> United Nations/North America (UN/NA) identification number, if applicable; <br /> 5) The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; -The transporter's name, address, and identification number; <br /> 6) The driver's signature; <br /> 7) A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> The CaIAG training record provided includes Spill Response Awareness and Emergency Procedures and is sufficient to <br /> fulfill the training record requirement.This violation will be closed. <br /> If you have any further questions or concerns, please contact me (209)468-3562. <br /> From: Denise Eichhorn <daesheller@yahoo.com> <br /> Sent:Tuesday,April 23, 2019 7:22 AM <br /> To: Christine Lawrence<clawrence@sjgov.org> <br /> Subject: Corrections of Violations <br /> Good morning, <br /> Attached are the corrected documents you asked for.
