San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.slgov.orq/ehd <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> RITE AID#6291 1 1970 W GRANT LINE RD,TRACY October 23, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 119 CCR 66265.16(d) Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste prior to 2018 were not available for review at the <br /> time of inspection. <br /> Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br /> employee training is properly documented and submit training records prior to 2018 to the EHD. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> The following was observed in the facility's HW storage area: <br /> -One out of the three cardboard boxes with Toxic/Poison HW labels, did not have the contents marked on the label <br /> (IMG 2780-2783).Jeffrey opened the box,which contained over the counter medicines, electronics, skin ointment, <br /> and toothpaste. Jeffrey marked the contents on the label and sealed the box(IMG 2786). <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -This facility has been changed to a RCRA Large Quantity generator, since the facility generated over 2.2 lbs of <br /> RCRA acute hazardous waste in September 2018, according to manifest 018821188JJK (9/13/18) (IMG <br /> 2764-2766).
