The current plan is lacking section C13 Emergency Coordinator
contact information.
The contingency plan must include:
1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or
non -sudden release of hazardous waste to air, soil or surface water at the facility;
2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and
State and local emergency response teams to coordinate emergency services;
3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency
coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and
all others in order in which they will assume responsibilities;
4. Immediately prepare or amend an existing plan to fulfill
the requirements of this chapter. Submit a copy of the contingency plan to the EHD.
This is a minor violation.
FA0021137 PR0536804 SCO01 07/15/2019
EHD 22-02 Rev. 09/27/2018 Page 5 of 9 Large Quantity Hazardous Waste Generator OIR
1868 E.
