San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#01240 678 N WILSON WAY, STOCKTON April 08, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste <br /> handling or the training provided has been inadequate. <br /> -A black crate was found in the hazardous waste (HW)storage area containing the following items(see photos IMG <br /> 5254-5, 5267): <br /> -An empty container of Fabuloso with Bleach Alternative Multi-Purpose Cleaner <br /> -An empty container of LA's Totally Awesome All Purpose Concentrated Cleaner and Degreaser Spot Remover <br /> -An empty container of Home Store Bleach was found adjacent the black crate in the HW storage area (IMG 5267). <br /> -An empty container of Fabuloso Multi-Purpose Cleaner was found on a shelf behind the HW totes(IMG 5265, <br /> 5267). <br /> -An empty container of Fabuloso Multi-Purpose Cleaner was found on the floor adjacent the HW storage area (IMG <br /> 5271-2). <br /> According to Shelly Felix, store manager,the items listed above were store-use items left by employees who used <br /> the items to clean the facility the night of 4/7/2019. Shelly stated the employees should have processed the empty <br /> containers as hazardous waste before leaving the facility last night, however instead the employees left the <br /> containers near the HW storage area for Shelly to process the next day. <br /> -An Aerosols, Flammables, and Combustibles(AF) HW tote was marked as only containing "Nail polish and <br /> remover"on the label, however the following items were found in the tote(IMG 5256-60, 5266, 5269-70): <br /> -An empty container of Tile Plus 4 In 1 Multi-Surface Cleaner. Shelly did not know what happened to the liquid that <br /> was once contained in the container, but indicated it may have been an item used by employees to clean the facility. <br /> When Shelly scanned the item, the hand-held scanner prompted her to"See guide"to determine which HW tote the <br /> item should be placed in. Shelly referenced the document titled "Products To Be Treated As Hazardous Waste," <br /> which was hanging on the HW storage area shelves. Shelly was unable to explain how it was determined that the <br /> empty container of Tile Plus 4 In 1 Multi-Surface Cleaner be placed in the AF HW tote. Shelly was not familiar with <br /> the StrongPak Resource Center phone number listed on the Dollar Tree Potentially Hazardous Material <br /> Management poster,which was posted adjacent the HW storage area, in case questions came up. Shelly was also <br /> not familiar with Dollar Tree's collection of electronic Material Safety Data Sheets(MSDS), available to the facility <br /> according to page 11 of Dollar Tree's California Training Guide on Hazardous Waste Handling and Processing <br /> Procedures <br /> -An aerosol can of Air Wick 6 in 1 Essential Oils Air Freshener <br /> -Six leaky/broken bottles of nail polish <br /> According to page 14 of Dollar Tree's California Training Guide, "as waste is placed in [a hazardous waste tote],the <br /> type of waste being added must be checked off the label." <br /> -A State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote was found <br /> containing the following (IMG 5261-5): <br /> -A bottle of Assured Hand Sanitizer.When Shelly scanned the item,the hand-held scanner prompted her to place <br /> the item into the AF HW tote. Shelly placed the sanitizer into the AF HW tote at the time of inspection. <br /> -Two glass prayer candles.When Shelly scanned the candles, the hand-held scanner prompted her to"See <br /> guide"to determine which HW tote the items should be placed in.
