SAN JOAQUIN COUNTY— Environmental OU 1TY Environmental Health Department Underground Storage Tank Program Inspection Report Facility Name: I Facility Address: Date: VALLEY PACIFIC FRESNO AVE CARDLOCK 1524 FRESNO AVE, STOCKTON January 17, 2020 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR- Notice to Comply) Item # Remarks 209 CCR 2716(a -e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The Do report of 8/8/2019 did not include the Do's signature and name - The Do report of 8/8/2019 did not include the date signed by the owner/operator and name of owner/operator. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. OBSERVATION: A copy of the current UST operating permit was not found on site. REGULATION GUIDANCE: A copy of the current UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: Immediately obtain a copy of the current permit and retain it on site. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. OBSERVATION: Liquid was observed in the under dispenser containment (UDC) #1 and #1/2 and Seeping on piping , indicating a leak in the primary containment. REGULATION GUIDANCE: All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by February 16, 2020.
