The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Aramark Uniform & Career Apparel , LLGStockton as of March 15 , <br /> 2019 . <br /> Open violations from December 28 , 2018 inspection <br /> Violation #603 - Failed to mark initial accumulation start date on satellite containers . <br /> One red metal 55 gallon drum of used oil did not have an initial start date marked . The last disposal record for used <br /> oil that was available during the inspection had a date of 3/9/2017 . The initial date of waste accumulation shall be <br /> clearly marked and visible for inspection on each container used for satellite accumulation of hazardous waste . <br /> Satellite containers of hazardous waste can stay on site up to one year. Since it cannot be determined how long this <br /> waste has been stored on site , immediately dispose of this waste and provide a copy of the hazardous waste <br /> manifest to the EHD . <br /> Violation #605 - Failed to completely label containers or portable tanks of hazardous waste . <br /> -One red 20 yard dumpster of filter cake was observed without a hazardous waste label . This was corrected on site . <br /> -One red metal 55 gallon drum of used oil was observed next to the filter cake dumpster without a label . This was <br /> corrected on site . <br /> -Six green plastic drums of shaker waste ( lint and other debris) were observed outside the northwest corner of the <br /> building without hazardous waste labels . The operator stated that this waste would be put inside the filter cake <br /> dumpster. <br /> -Two black metal 55 gallon drums of hazardous waste were observed outside between the empty plastic drums and <br /> totes on the northwest corner of the building with faded , unreadable labels . <br /> All hazardous waste containers shall be marked with the following information : <br /> - the words " Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state <br /> - composition (contents) <br /> - accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information . <br /> Open violations from May 13 , 2015 inspection <br /> Violation #102 - Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br /> years . <br /> Four 55-gallon poly containers , three 55-gallon red metal containers , six 5-gallon plastic containers , and two <br /> 5 -gallon open containers (with some black oily liquid ) were located adjacent to the chemical storage area , were <br /> unlabeled , and at the time of inspection lacked a hazardous waste determination . Any person who generates a <br /> waste shall determine if the waste is a hazardous waste . Immediately make a hazardous waste determination for <br /> the noted containers and manage them according the Title 22 hazardous waste regulations . Submit a statement <br /> and supporting documentation explaining how this waste was managed . <br /> Violation #104 - No modified contingency plan . <br /> An emergency coordinator and modified contingency plan information is lacking . There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures , and the following <br /> information must be posted by a phone : the name and phone number of the emergency coordinator; location of fire <br /> extinguishers , spill control equipment , and if present, fire alarm ; and the phone number of the fire department, <br /> unless the facility has a direct alarm . Immediately appoint an emergency coordinator and post the required <br /> information by a phone . A form is provided that can be used for this purpose . <br /> i <br /> Page 5 of 6 <br />
