(see photo IMG 7694) <br /> There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, <br /> and the following information must be posted by a phone:the name and phone number of the emergency coordinator; <br /> location of fire extinguishers, spill control equipment,and if present, fire alarm; and the phone number of the fire <br /> department, unless the facility has a direct alarm. <br /> Immediately post the required information by a phone. <br /> A form is provided that can be used for this purpose. <br /> This is a repeat violation, Class II. <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the following observations, the facility employees are not trained on proper hazardous waste(HW)handling <br /> or the training provided has been inadequate. <br /> 1.According to Jeffrey Delgado,Assistant Store Manager,when employees are unable to find or make a hazardous <br /> waste determination for a waste item, based on the information provided on the hand-held item scanning system, and <br /> the Dollar General Environmental Compliance poster and guidance documents posted at the hazardous waste storage <br /> area,employees will use their best judgment to make a hazardous waste determination.Jeffrey was not aware of who <br /> or the phone number to call when he has questions about a waste item's hazardous waste determination.Jeffrey was <br /> not aware of the phone number to call to access item Safety Data Sheets(SDS),which was found on the Dollar <br /> General Safety Information Center poster in the employee break room. (IMG 76534,7695-6) <br /> 2.When asked how employees clean up chemical spills and manage the waste generated from those spills, Jeffrey <br /> stated spill magic is used to absorb a liquid chemical spill,which is then swept up and thrown into the trash.Jeffrey <br /> stated that if the chemical container can no longer be sold or used by the facility, the chemical container will be <br /> scanned using the hand-held scanner, placed in a bag, and then placed into the appropriate hazardous tote. <br /> According to the Dollar General Environmental Compliance Program Training guide,found in the hazardous waste <br /> storage area, "absorbent pads used during the clean up and any other clean up materials"should be placed into a <br /> re-sealable bag and then placed into the"correct[hazardous waste]tote(i.e. Acid,Alkaline, Flammable, Oxidizer or <br /> Toxic)."Jeffrey stated the used spill magic is thrown into the trash because he thought the spill magic neutralized <br /> chemicals.
