If the generator did not receive a copy of the manifest with the <br /> handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 <br /> days of the date the waste was accepted by the initial transporter,the generator shall submit a legible copy of the <br /> missing manifest,with some indication that the generator has not received confirmation of delivery. This information <br /> shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. <br /> Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -The amount of hazardous waste generated in 2018 could not be determined at the time of inspection because <br /> manifest 005888175FLE (5/16/18)found on site was not legible and was not on file with DTSC. <br /> -The facility has two different active EPA ID numbers: CAL000373076 and CAR000278317. It is recommended <br /> for the facility to inactivate one of its EPA ID numbers. <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report which replaces the initial checklist on 8/2/2019. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 9/1/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
