Steve Bohnak, District Manager, indicated the container was <br /> likely left there illegally by an unknown individual. Immediately make a hazardous waste determination for this <br /> container and manage per Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> -Steve Bohnak, District Manager, requested the following list of the manifest numbers that were not found on site <br /> at the time of inspection: <br /> - 011388660FLE (8/17/18) <br /> -008631598FLE (10/20/16) <br /> The following manifests were found on site without a signed off copy from the destination facility, and were <br /> provided to Steve at the time of inspection: <br /> - 013120796FLE (6/26/19) <br /> -013120797FLE (6/26/19) <br /> -013120479FLE (4/21/19) <br /> -013120478FLE (4/22/19) <br /> -011412830FLE (10/19/18) <br /> -008548716FLE (6/7/17) <br /> -008680341 FLE (11/13/17) <br /> An inspection checklist was provided to the facility operator at the time of inspection.
