sr`� JOAOUP'i <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: FDat,Dollar General#14353 915 E Yosemite Ave, Manteca 1, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> 1.When asked how employees determine which of the differently categorized HW totes a waste item belongs in, <br /> Tenisha Arbet, Store Manager, said each item is scanned using a hand-held scanner,which will sometimes state <br /> which HW tote the item belongs in. Tenisha indicated that when the scanner does not specify which HW tote an item <br /> belongs in, employees will read the item's label and ingredients, as well as the PSC HW guidance poster and <br /> documents,which provide product examples and key words for each of the HW categories, to determine which HW <br /> tote an item belongs in.When asked if a determination cannot be made based on the information on the item label <br /> and the guidance poster and documents,Tenisha stated she will contact her district manager. Steve Bohnak, District <br /> Manager, indicated he would call Eric Voyles, Dollar General Director of Facilities Management, if needed,to ask for <br /> assistance on making the determination. Tenisha and Steve did not seem aware of Dollar General's Respond ticketing <br /> system to submit a notification to DG Corporate regarding the scanning system not providing a HW category for a <br /> particular waste item. <br /> 2.A HW tote containing flammable HW and labeled Flammable, was also labeled as containing Oxidizer HW.Tenisha <br /> Arbet, Store Manager, replaced the Oxidizer label with a Flammable label at the time of inspection. (IMG 7815-7) <br /> 3. The waste items listed below were found in a bag, containing two containers of hair conditioner and make-up, and <br /> DG(Dollar General) Eye Make-Up Remover Pads, inside a Toxic HW tote(IMG 7819-20, 7824-27); <br /> -A bottle of LA Colors Calor Last nail polish,which had a flammable warning label. <br /> -A Duracell Cell Phone Car Charger <br /> -A Wireless Gear AC Charger <br /> According to the DG Environmental Compliance Program Training, and a PSC HW guidance poster and documents, <br /> found in the HW storage area, flammable items are to be placed in a Flammable HW tote and electronic products are <br /> to be placed in a Universal Waste tote. <br /> 4. The waste items listed below were found bagged and Inside the Oxidizer HW tote(IMG 7830-41): <br /> -A bottle of Top Job Bleach, an oxidizer, according to the Oxidizer HW tote label. <br /> -Three containers of DG health Children's Allergy medication <br /> -A bag filled with various make-up items,two light bulbs,Art Skills Poster Lights(electronic), LA Colors Lash <br /> Adhesive, and two bottles of flammable nail polish. <br /> According to the Toxic HW tote label, over-the-counter Health&Beauty(including cosmetics), medicine, and light <br /> bulbs are to be placed in a Toxic HW tote. Steve Bohnak, District Manager, stated that he believed many adhesives or <br /> glues were flammable. Tenisha pointed to a photo of an adhesive called Goop on the PSC guidance document for <br /> Flammable HW.Also, Super Glue was found in a Flammable HW tote(IMG 7815-7818), <br /> 5.The waste items listed below were also found bagged and inside the Oxidizer HW tote(IMG 7842-6); <br /> -A bag containing Disney Pixar Character Lights(electronic), five Right Guard Xtreme Defense deodorant sticks, <br /> and five boxes of Xyxal Allergey medication. <br /> When Steven scanned the deodorant sticks and the allergy medication,the hand-held scanner stated these items <br /> should be placed into the Toxic HW tote. <br /> 6. The waste items listed below were also found bagged and inside the Oxidizer HW tote(IMG 7849-55, 7857, 7859, <br /> FA0021712 PR0537831 SCO01 07/31/2019 <br /> EHD 22-01 Rev 09/27/2018 Page 5 of 14 Small Quantity Hazardous Waste Generator OIR <br />
