`5AN 10AHWI <br />Small Quantity Hazardous Waste Generator Inspection Report <br />FacilityName: <br />Facility Address: <br />ty <br />Date: <br />Dollar General #14353 <br />915 E Yosemite Ave, Manteca <br />Julv 31. 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />103 <br />CCR 66262.40(c) Failed to retain hazardous waste determination including waste analysis on site for 3 years. <br />When asked how employees determine which of the differently categorized hazardous waste (HW) totes a waste item <br />belongs in, Tenisha Arbet, Store Manager, said each item is scanned using a hand-held scanner, which will <br />sometimes state which HW tote the item belongs in. Tenisha indicated that when the scanner does not specify which <br />HW tote an item belongs in, employees will read the item's label and ingredients, as well as the PSC HW guidance <br />poster and documents (posted in the HW storage area), which provide product examples and key words for each of <br />the HW categories, to determine which HW tote an item belongs in (see photos IMG 7815, 7819, 7830, 7862, 7875, <br />7901, 7923). When asked if a determination cannot be made based on the information on the item label and the <br />guidance poster and documents, Tenisha stated she will contact her district manager. Steve Bohnak, District <br />Manager, indicated he would call Eric Voyles, Director of Facilities Management, if needed, to ask for assistance on <br />making the determination. Tenisha and Steve did not seem aware of Dollar General's Respond ticketing system to <br />submit a notification to Dollar General (DG) Corporate regarding the scanning system not providing a HW category for <br />a particular waste item. <br />The waste items listed below were observed in the HW totes, but at the time of inspection it could not be determined <br />how the HW determinations for each of these items were made. The hand-held scanner did not specify which HW tote <br />the items should go in, and the PSC guidance poster and documents did not list these types of items: <br />1. A bag of Hickory Charcoal Briquets was found in a Toxic HW tote (IMG 7819-23). <br />2. A bag of six damaged Citrus Zest candles was found in a Oxidizer HW tote (IMG 7830-1, 7847-8). <br />3. DG Beauty Eye make-up Remover Pads were found in a Toxic HW tote and in an Oxidizer HW tote (IMG 7819-20, <br />7827-9, 7830-1, 7854-55). <br />4. An L.A. Colors Lash Adhesive was found in a Toxic HW tote (IMG 7819-20, 7836-7). Steve Bohnak, District <br />Manager, stated that he believed many adhesives or glues were flammable. Tenisha pointed to a photo of an adhesive <br />called Goop on the PSC guidance document for Flammable HW. <br />5. One brand of Super Glue was found in a Flammable HW tote, while another brand of Super Glue, along with a clear <br />Gorilla Glue, were found in an Alkaline HW tote (IMG 7815-7818, 7862-3, 7888, 7891). <br />6. A bottle of Gain In -Wash Scent Booster and two bottles of Downy In -Wash Scent Beads were found in a Universal <br />Waste tote (IMG 7909, 7920-1). <br />7. A 7 -Ib bag of Forever Pals Cat Litter was found in a Universal Waste tote (IMG 7909, 7922). <br />A generator shall make a hazardous waste determination and keep a record of any test results, waste analyses, or <br />other determinations made in accordance with hazardous waste regulations for at least three years from the date that <br />the waste was last disposed of. <br />Immediately locate a copy of the hazardous waste determination made for the hazardous waste items listed above, <br />ensure store employees are familiar with these waste determinations, and submit a copy to the EHD. <br />If a copy is unavailable, immediately make a hazardous waste determination and begin managing the above listed <br />waste under Title 22 hazardous waste regulations.
