SAN a0AQIJIN <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />12150 <br />Date: <br />I <br />Dollar General #13585 <br />W Alpine Ave, Stockton <br />Au4ust 06, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />106 <br />CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />1. When asked how employees determine which of the differently categorized hazardous waste (HW) totes a waste <br />item belongs in, Julie Sullivan, Store Manager, said each item is scanned using a hand-held scanner, which will <br />sometimes state which HW tote the item belongs in. Julie indicated that when the scanner does not specify which <br />HW tote an item belongs in, employees will read the item's label and ingredients, as well as the Dollar General (DG) <br />HW guidance poster and documents, which provide product examples and key words for each of the HW categories, <br />to determine which HW tote an item belongs in (see photos IMG 8049, 8051-7). When asked what employees do if <br />they are still unsure about which HW tote a waste item belongs in, Julie stated she will place the item into the Toxic <br />HW tote. Julie did not know who to call in case she had questions about an item's hazardous waste determination. <br />Additionally, Julie was not aware of Dollar General's (DG) Respond ticketing system to submit a notification to DG <br />Corporate regarding the scanning system not providing a HW category for a particular waste item. <br />2. The waste items listed below were found bagged and inside a tote labeled Toxic HW (IMG 8007, 8010-15): <br />- Approximately seven containers of AeroScent Ocean Breeze Odor Eliminator air freshener. According to the Dollar <br />General (DG) Flammable HW guidance document, air fresheners/fragrances are to be placed in the Flammable HW <br />tote. <br />- A DG Home Scented Oil Warmer with Refill, and two packages of two Air Wick plug in scented oil refills. According <br />to the Dollar General (DG) Flammable HW guidance document, air fresheners/fragrances are to be placed in the <br />Flammable HW tote. <br />3. The waste items listed below were found bagged and inside a tote labeled Flammable HW (IMG 8016-24): <br />- Two cardboard boxes containing wax candles, approximately seven to eight candles total, and one brand of which <br />was called True Living in Vanilla Sugar scent. It could not be determined how the HW determination for the candles <br />was made, since the scanner did not indicate which HW tote the candles should be placed in, and the DG HW poster <br />and guidance documents did not list candles or a similar type of item. Also, a wax candle was found in a Toxic HW <br />tote (IMG 8025, 8044-5). <br />- Three 96 fl oz containers of Top Job Bleach. According to the DG Oxidizer HW document, bleach is to be placed in <br />an Oxidizer HW tote. <br />4. The waste items listed below were found bagged and inside a tote labeled Alkaline HW (IMG 8028-30): <br />- Three containers of Swiffer Sweeper Wet Clothes. According to the hand-held scanner, these wet clothes should be <br />placed into the Toxic HW tote. <br />5. The waste items listed below were found bagged and inside a tote labeled Toxic HW (IMG 8031-45): <br />- A container of DG Home Oxy Shower, Tub & Tile Cleaner that "powers through grime, soap scum, calcium & lime" <br />according to the item's label. According to the DG Acid HW tote, calcium/lime/rust removers and toilet/tub/tile <br />cleaners are to be placed in the Acid HW tote. <br />- An LA Colors nail polish that is flammable according to the item's label. According to The DG HW guidance poster, <br />flammable items are to be placed in the Flammable HW tote. <br />- Three boxes of DG Health Coated Nicotine Gum. According to the DG HW guidance poster and Smoking Cessation <br />guidance document, nicotine gum is to be placed in the Smoking Cessation tote. <br />- Approximately five individually packaged Bowl Fresh (toilet) Bowl Cleaner and Air Fresheners.
