The following manifest numbers were found without a signed off copy from the destination facility: - 011386185FLE (2/26/19) (IMG 8059) - 011392658FLE (8/24/18) (1MG 8063) - 010868404FLE (4/4/18) (IMG 8062) - 010076706FLE (1/12/18) (IMG 8065) Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. This is a Class II violation. 405 CCR 66262.34(d)(2) Failed to manage incompatible wastes in containers properly. 1. The waste items listed below were found bagged and inside a tote labeled as and containing Flammable HW (IMG 8016-24): - Three 96 fl oz containers of Top Job Bleach. According to the DG Oxidizer HW document, bleach is to be placed in an Oxidizer HW tote. Incompatible wastes shall not be placed in the same container, or in an unwashed container that previously held an incompatible waste or material. A container holding a hazardous waste that is incompatible with any waste or other materials transferred or stored nearby in other containers, piles, open tanks, or surface impoundments shall be separated from the other materials or protected from them by means of a dike, berm, wall, or other device. Immediately transfer or separate the oxidizer and flammable hazardous waste to other containers or separate the containers. This was corrected on site. This Is a Class II violation.
