Also, according to the DG Flammable HW guidance document, air fresheners are <br /> to be placed in the Flammable HW tote. <br /> -Five seemingly empty containers of liquid laundry detergent,four of which had broken caps. There was no spill <br /> clean-up material found to account for the liquid missing from the five containers, and Julie Sullivan, Store Manager, <br /> did not know what happened to the missing liquid. <br /> -A wax candle. It could not be determined how the HW determination for the candle was made, since the scanner did <br /> not indicate which HW tote the candle should be placed in, and the DG HW poster and guidance documents did not <br /> list candles or a similar type of item.Also wax candles were found in a Flammable HW tote(IMG 8016-9). <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. <br /> Provide proof of re-training and training materials to the EHD for employees whose responsibilities include hazardous <br /> waste. <br /> This is a Class II violation. <br /> 109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> According to Jose Lomeli, District Manager, manifests received by the facility for the disposal of hazardous waste are <br /> kept in a binder in the hazardous waste storage area, and copies of the manifests are not sent or mailed anywhere. <br /> Copies of the following manifests numbers were observed on site, but are not on file with DTSC: <br /> -011386186FLE(2/25/19)(IMG 8060) <br /> -011386185FLE(2/26/19)(IMG 8059) <br /> -011392658FLE(8/24/18)(IMG 8063) <br /> -010076706FLE(1/12/18)(IMG 8065) <br /> The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either the <br /> generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O.
