A J OA U I Environmental Health Department <br /> COUNTY <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SHELL 1-5 1 717 W EIGHTH ST, STOCKTON March 12, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. <br /> OBSERVATION: A copy of the UST operating permit was not found on site. <br /> REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Dispenser#7/8 and#6/7 shows signs of seepage in the center product pipe and signs of slight <br /> dripping, indicating a leak in the primary containment. <br /> REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a minor violation. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> OBSERVATION: Annual spill container testing was previously performed on 3/2018 and was completed on 11/2019, <br /> 8 months past due. <br /> REGULATION GUIDANCE: This test is required upon completion of installation and every 12 months thereafter. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by April 11, 2020. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
