The following is an itemized list of underground storage tank violations that have not <br /> been addressed for TOKAY SHELL as of October 07, 2020. <br /> Open violations from May 07, 2020 inspection <br /> Violation#102-UST operating permit application for facility and tank information not submitted or current. <br /> OBSERVATION: UST Tank Information form for the used oil tank is not current in the California Environmental <br /> Reporting System (CERS). <br /> -The site is using audible/visual alarms for overfill protection however, according to the tank form the site does not <br /> have ball floats. <br /> -The tank form incorrectly states that the product/waste lines are single walled. <br /> -The tank form incorrectly states there are no spill containers and striker plate present. <br /> -Corrosion protection information is left blank. <br /> -Vent and riser piping information is left blank. <br /> REGULATION GUIDANCE:Any change of information must be updated in CERS within 30 days of the changes. <br /> CORRECTIVE ACTION: Immediately log into CERS, update the required information, and submit for review by the <br /> EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#104-Failed to have an approved UST monitoring plan. <br /> OBSERVATION: The monitoring plan is not current and/or not approved by the EHD. <br /> -The used oil monitoring plan incorrectly states the the annular sensor to be a Veeder-Root 302 sensor. <br /> Veeder-Root 409 sensor was observed onsite. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 1 of 8 <br />
