Hazelton Ave <br /> Stockton, CA 95205 <br /> Re: Dollar Tree#1260 <br /> 3728 East Hammer Lane <br /> Stockton, CA 95212 <br /> Dear Ms. Florido, <br /> In response to the inspection conducted on May 6, 2019 and the amended report of May 16, 2019, for <br /> Store#01260 in Stockton, California, please see the outline below, and the exhibits which follow, <br /> referencing existing procedures, documentation and/or actions taken: <br /> • A Dollar Tree EH&S Specialist retrained the Store Management team on the Company's <br /> Hazardous Waste Program. <br /> They were coached on the proper identification and management of hazardous waste including <br /> the use of available resources and who to call for support if unsure. All totes containing waste <br /> were assessed for proper categorization, processing and labeling. It was emphasized the <br /> damaged hazardous products must be taken to the accumulation area immediately and <br /> processed, not left in other areas of the store. <br /> The Store Manager and two Assistant Store Managers were trained/retrained on May 22, 2019 <br /> (EXHIBIT A). <br /> • The Emergency Coordinator role and supporting documentation was reviewed with the Store <br /> Manager. Her name and contact information were added to the Emergency <br /> Response/Contingency Plan as well as the Emergency Contacts/FE Locations template <br /> provided by the inspector. The SJ County form and Emergency Response/Contingency Plan <br /> was posted at the phone to be accessible for all associates (EXHIBIT B). <br /> • Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br /> company and has resubmitted the manifests.
