SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#01260 3728 E HAMMER LN STOCKTON May 06 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> -A STOU HW tote was found with the contents not marked on the tote's HW label.The STOU HW tote contained the <br /> following (IMG 5927, 5931-8): <br /> o A HW bag labeled"CB"and containing a partially empty 32 fl. oz. container of LA's Totally Awesome Oxygen <br /> Orange All-Purpose Degreaser.The scanner indicated the degreaser should be placed in a CB HW tote. Somphon did <br /> not know what happened to the missing liquid that was once in the container. Used absorbent was not found to <br /> account for the missing liquid. Somphon placed the degreaser into a CB HW tote. <br /> o A HW bag containing a box of Color Eazy Permanent Cream Hair Color,which contained a tube of permanent hair <br /> color and a bottle of developer. When the barcode on the box was scanned,the scanner indicated the items in the box <br /> should be placed in an STOU HW tote. However,the ingredient list on the box stated that the cream hair color <br /> contains ammonium hydroxide and the developer contains hydrogen peroxide.According to the Dollar Tree California <br /> Training Guide on Hazardous Waste Handling and Processing Procedures items containing ammonium hydroxide are <br /> classified as CB and items hydrogen peroxide are classified as OX. The hair color and developer were not placed in <br /> separate HW totes at the time of inspection. <br /> -A STOU HW tote was found containing several items, however the contents was not marked on the tote's HW label. <br /> One of the items in the tote was a HW bag labeled "STOU"containing a Hybrid Pocket Lighter.The scanner indicated <br /> the lighter should be placed in an AF HW tote. Somphon placed the lighter into the AF HW tote. (IMG 5940-3) <br /> -An open cardboard box containing the following waste was found on a stack of product boxes in the facility's back <br /> storage area: <br /> o A partially empty 22 fl. oz. container of Fabuloso multi-purpose cleaner <br /> o A partially filled 1.8 oz aerosol can of Goofy String,which was missing the actuator <br /> o A broken candle <br /> Somphon did not know how long ago these items had been placed in the facility's back storage area. Somphon did <br /> not process these waste items and place them into HW totes at the time of inspection. <br /> -A sales shelf,which held containers of Comet and Ajax bleach-containing cleaning powders,was found with spillage <br /> of the bleach-containing powders(IMG 5950-4). <br /> -A sales shelf,which held containers of Ty-D-Bol toilet cleaner,was found with dried up spillage of the Ty-D-Bol <br /> cleaner(IMG 5955-7). <br /> -Somphon provided a record of employee Hazardous Materials Training,which Somphon stated includes hazardous <br /> waste training.The following employees were listed as having completed the iLearn Lesson: Somphon Panyasy(also <br /> appeared to be signed off by Julia Giddens, Dollar Tree Environmental Compliance&Safety Specialist, as having <br /> completed the Training Guide), Janet Bun, Lacie Geiszler, Elda Perez, Belinda Alcazar, Delicate Tyrell, Jessica Imes, <br /> Karla Villalona, Monica Barron, Maria Perez-Hernandez, Sukhjinder Kandola, Sasirega S., Thamoly Kerm, and <br /> Melissa Capuchin.The following employees were listed as not having completed the iLearn Lesson, nor the Training <br /> Guide: Eann Say,Tanya Martin, Michael Tate, Basilio Gines, Davena Suy, Sally Hing-Chham, Giselle Hernandez, and <br /> Anesimo Riveral Jr. <br /> The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br /> handling and emergency procedures. <br /> FA0015031 PRO538094 SCO01 05/06/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 7 of 18 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
