The designated operator failed to check and note the correct test dates for the secondary containment test on the 4/30/18, 5/21/18 and 9/20/18 designated operator monthly inspection report.The date listed in the DO reports stated 2/24/16 however the test was performed on 10/14/15 and 2/24/16. During the monthly inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class It. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The under all dispenser containment(UDC)appeared to be previously be monitored with floats and chains however 208 sensors were observed during today's inspections.The floats and chains have not been removed from the UDCs, and in the monitoring plan it list a 208 sensor as means of monitoring.The UDCs have not been properly converted to allow the 208 to detect a leak at the earliest opportunity.The UDCs have dividers at the bottom of the pans for float and chains, preventing the 208 sump sensor located in UDC 1/2, 3/4,5/6 and 7/8 located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Immediately contact a properly licensed,trained,and certified contractor to address under permit and inspection of the EHD, if required. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a Class It violation. 320 CCR 2635,2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill prevention. Owners or operators of underground storage tanks that do not meet California Code of regulations,title 23 division 3,chapter 16 section 2635(c)(2)shall test overfill prevention equipment once by October 13th,2018 and every 36 months thereafter;and within 30 days of the date of a repair.The site did not performed the testing by October 13th, 2018. Ensure to conduct overfill prevention equipment inspection within the time frame provided by the regulation and submit test results within 30 days of performing the inspection. Ensure a 48 hour notification is provided to EHD prior to conducting the inspection. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by November 24th, 2018 . Document reviewed: Monitoring system certification test results from 2015, 2016, 2017, secondary containment testing from 2015, employee training, response plan, maintenance records, monitoring plan, financial responsibility documents and designated operator monthly inspection reports from October 2017-September 2018.
