The designated operator failed to check and note the correct test dates for the monitoring system certification, spill containment, and leak detector testing on the June, July, and August 2018 designated operator monthly inspection reports. Designated operator marked 5/15/2017 instead of 2/21/2017.The designated operator failed to check and note the correct test date for the Secondary Containment testing on the January 2018 designated operator monthly inspection report. Designated operator marked 3/16/2016 instead of 3/3/2016. During the monthly inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to document all the alarms from the attached alarm history on the January 2018 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: L13 Diesel Fill Fuel alarm on 1/8/2018 and two L14 Supreme Fill Fuel alarms on 1/9/2018. During the monthly inspection,the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 313 HSC 25290.1,25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. In dispenser 3/4, liquid observed in the under containment dispenser and weepage observed on product piping, indicating a leak in the primary piping. All primary containment for the UST system must be tight. Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by March 15, 2018. Documents reviewed by regulatot: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer, the Owner Statement of Designated Operator Compliance and monthly reports (12 months), UST Monitoring Plans, Operating Permit, alarm history and setup, maintenance records, and employee training log. Documents received by facility: Return to compliance certification form and routine inspection report.
