(AGE) has prepared this report for the destruction <br /> of fifteen wells at 4696 East Waterloo Road (formerly 4648 East Waterloo Road), Stockton, <br /> California. The site location is depicted in Figure 1; site structures and former well locations <br /> are depicted in Figure 2. <br /> INTRODUCTION/BACKGROUND <br /> Eleven single completed, two-inch diameter, ground water monitoring wells (MW-1 and <br /> MW-5 through MW-14), three single completed, four-inch diameter, ground water <br /> monitoring wells (MW-1, MW-2, and MW-14), and one single completed, four-inch <br /> diameter, soil vapor extraction well (VW-1) had been installed on- and off-site as part of <br /> a subsurface investigation of the former underground storage tank (UST) area at 4696 <br /> East Waterloo Road (formerly 4648 East Waterloo Road), Stockton, California. The total <br /> depths and screen intervals of the wells are summarized in Table 1. <br /> PROCEDURES <br /> On 21 September 2011, a total of fifteen (15)wells were destroyed on- and off-site under <br /> San Joaquin County Environmental Health Department (EHD) well destruction permits <br /> (Appendix A). Well destruction activities were performed by All Well Abandonment, Inc. <br /> The wells were destroyed by grouting the well casings under pressure and over-drilling <br /> each well to three feet below surface grade (bsg).
