The following is an itemized list of hazardous materials violations that have not been addressed for DINO AND SON DITCHING SERVICES INC as of Augustl9, 2020. Open violations from July 14, 2020 inspection Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. Documentation showing initial and annual refresher training of 1. Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency; 2. Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment; and 3. Evacuation plans and procedures, including immediate notice, for the business site; was not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures, the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
