San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax:(209)468-3433 Web:www.sogov.org/ehd <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Dollar Tree#05403 1209 E March Ln, Stockton April 09, 2019 <br /> Item# I Remarks <br /> 102 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> A Dollar Tree basket containing 38 bottles of Nature's Measure Glucosamine Joint Therapy tablets, 31 of which had <br /> a best buy date of 7/7/2018 and 7 of which had a best buy date of March 2019,was found in an area of the stock <br /> room located behind the hazardous waste (HW)storage area (see photos IMG 5306-8, 5311, 5313, 5317).The <br /> bottle labels provided Supplemental Facts.According to Annalyn Mamaradlo, store manager, she suspected an <br /> employee removed the bottles from the sales shelf since they were expired, but did not determine what to do with <br /> the bottles.According to page 13 of Dollar Tree's California Training Guide on Hazardous Waste Handling and <br /> Processing Procedures, "any waste product whose label provides"Drug Facts"or"Supplemental Facts" must be <br /> managed as State-Regulated,Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU), regardless of <br /> the product's ingredients."Annalyn placed the bottles into two hazardous waste bags and into an STOU HW tote <br /> (IMG 5316, 5318). <br /> An empty container of LA's Totally Awesome Bleach was found on the ground adjacent the facility's mop sink. <br /> Annalyn scanned the barcode on the bleach and the hand-held scanner prompted her to place the item into the <br /> Oxidizers(OX)HW tote(IMG 5327-30).Annalyn placed the empty bleach container in a bag and into the OX HW <br /> tote. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a <br /> hazardous waste determination for any potential hazardous waste, and manage it according the Title 22 hazardous <br /> waste regulations. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 104 CCR 66262.34(d)(2)No modified contingency plan. <br /> The modified contingency plan posted in the facility office listed Eric Darville as the emergency coordinator, however <br /> according to Annalyn Mamaradlo, store manager, Eric no longer works at the facility(IMG 5333). <br /> There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, <br /> and the following information must be posted by a phone:the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present,fire alarm; and the phone number <br /> of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the required information by a phone. <br /> This is a Class II violation. <br /> FA0022071 PR0538317 SCO01 04/09/2019 <br /> EHD 22-01 Rev.09/22/16 Page 4 of 12 Small Quantity Hazardous Waste Generator OIR <br />
