Hazelton Ave <br /> Stockton, CA 95205 <br /> Re: Dollar Tree#05193 <br /> 10408 Trinity Parkway <br /> Stockton, CA 95219 <br /> Dear Ms. Florido, <br /> In response to the inspection conducted on May 8, 2019 and the amended report of May 20, <br /> 2019, for Store#05193 in Stockton, California, please see the outline below, and the exhibits <br /> which follow, referencing existing procedures, documentation and/or actions taken: <br /> • A Dollar Tree EH&S Specialist retrained the Store Management team on the Company's <br /> Hazardous Waste Program. <br /> They were trained on the proper identification and management of hazardous waste. All <br /> totes containing waste were assessed for proper categorization, processing and labeling. It <br /> was emphasized that chemical items used by the store must be labeled with a store-use <br /> sticker. <br /> The Store Manager, an Assistant Store Manager, and the Freight Manager were <br /> trained/retrained(EXHIBIT A). <br /> • Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br /> company and has resubmitted the manifests. Please see the vendor letter of responsibility <br /> (EXHIBIT B). <br /> • Copies of the generator manifests, though previously submitted by Stericycle, were re- <br /> submitted to DTSC (EXHIBIT C). <br /> • Totes in the Hazardous Waste Accumulation Station(EXHIBIT D)have been properly <br /> labeled and kept closed(EXHIBIT E).
