Time In: 3:00 pm <br /> Time Out: 3:51 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: Tiwana&Sons Inc dba Subway# 15879 Date: 04/28/2020 <br /> Address: 4410 E WATERLOO RD, STOCKTON 95215 <br /> Owner/Operator: Telephone: (209)482-2580 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00051890 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> EMPLOYEE HAS PINK EYE (OR EYES THAT ARE VERY WATERY AND RED)AND KEEPS RUBBING HER EYES <br /> WHILE MAKING FOOD COMPLAINANT WOULD LIKE A CALL BACK. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival it spoke wit a male associate who stated he was in charge at this time. I explained the reason for my visit and <br /> observed the other two staff working at that time. The other two associates were woman, both wearing mask and hats, <br /> making it difficult to fully see their eyes. Before I spoke to anyone I waited in line as a customer and observed the staff <br /> prepare food, during this time I observed none of them rubbing their eyes or observed their eyes giving them any issues.After <br /> I spoke with the staff i asked for them to remove their hats and mask in order to see if any of them had any visible symptoms <br /> or pink eye or irritation of the eyes. None of them contained any visible symptoms and one of the staff members stated to me <br /> that they have allergies and when the front door opens, her allergies do effect her eyes and make them watery or itchy.
