There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, <br /> spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has <br /> a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form <br /> is provided that can be used for this purpose. <br /> This is a Class II violation. <br /> 105 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br /> hazardous waste were properly trained. Mai Trinh, Outpatient pharmacy manager, stated she had not received <br /> hazardous waste training. She stated she would throw Coumadin and Warfarin (P-listed hazardous waste) containers <br /> in the trash. She was not aware of RCRA or NON-RCRA wastes. There was a "Proper Waste and Linen Segregation <br /> & Disposal by Container Type"chart which listed the container hazardous waste should be placed in, above the blue <br /> disposal container, that was being used for dropped and broken pills. The generator must ensure that all employees <br /> who handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. <br /> Immediately provide training to all employees who handle hazardous waste and submit a copy of the training records <br /> to the EHD. <br /> This is a Class II violation. <br /> 106 CCR 66262.20 Failed to prepare manifests for transportation of hazardous waste. <br /> Hazardous waste was transported off site, to 525 W. Acacia St., Stockton CA for disposal without first preparing a <br /> uniform hazardous waste manifest. A generator shall prepare a Uniform Hazardous Waste Manifest on EPA Form <br /> 8700-22 prior to the transport of a hazardous waste for off-site transfer, treatment, storage or disposal. Ensure that a <br /> manifest is prepared each time hazardous waste is transported for off site transfer, treatment, storage or disposal. <br /> This is a Class II violation. <br /> 109 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste were not found on site. Hazardous waste generators <br /> shall retain copies of all manifests signed off by the disposal facility on site for three years and have them readily <br /> available for review.
