ADDENDUM TO HEALTH AND SAFETY PLAN FOR GROUNDWATER WELL <br /> INSTALLATION AND SAMEPLING ACTIVITIES <br /> AT THE TRACY TIRE FIRE SITE <br /> SAN JOAQUIN COUNTY, CALIFORNIA <br /> August 15, 2003 <br /> 003-09025-00 <br /> INTRODUCTION <br /> This addendum, when included with the attached Health and Safety Plan (HSP) for the <br /> Tracy Tire Fire Site, located in San Joaquin County, California ("the Site"), represents an <br /> updated HSP for the Site. The HSP for the Site was prepared by LFR Levine-Fricke (LFR) <br /> and was dated February 2003. No significant changes to the physical and chemical hazards <br /> have been identified at the Site and all hazard information and personnel safety <br /> requirements contained in the HSP remains in force. Sections in the HSP should be <br /> replaced with the following: <br /> Section 3 —Planned Site Activities <br /> Scheduled work will consist of the following activities: <br /> ® Conduct soil sampling activities in tire pile burn areas after apparent pyrolytic oil <br /> and/or burn material has been removed by excavation and scrapping. <br /> Work is anticipated to begin during September 2003. <br /> Section 4— Key LFR Levine Fricke Personnel and Responsibilities <br /> Site Safety Officer James Eisert <br /> Project Manager James Eisert <br /> Corporate Director, Health and Safety Jim Bucha, CIH <br /> The responsibilities of the LFR project personnel are outlined in the HSP. <br /> ACTION LEVELS <br /> The following action levels were developed for exposure monitoring with real-time air <br /> monitoring instruments. The air monitoring data will determine required PPE levels at the <br /> Site during scheduled intrusive activities. The action levels are based on sustained readings <br /> indicated by the instrument(s). Air monitoring will be performed and recorded at up to 30- <br />
