sb <br /> 10.3.3 Subcontractors: <br /> The training program will duplicate the training for supervisors and employees, when those <br /> subcontractors enter an exclusion zone. Length of training may vary slightly at the <br /> discretion of the CIWMB Engineer, on the basis of the individual's previous training and <br /> extent of potential chemical and physical hazards. <br /> 10.4 Refresher Training <br /> An 8-hour HAZWOPER annual refresher course is required for those individuals having already <br /> received their initial 40-hour course requirements. This training is required every twelve <br /> months. If an individual misses his/her assigned training and exceeds the annual requirement, <br /> the individual will not be allowed to work on this project until such time that refresher training <br /> has been completed. In addition, if an individual fails to receive refresher training with 24 <br /> months II year grace period), the entire 40-hour HAZWOPER course must be conducted again <br /> prior to working on this or any other HAZWOPER site. Refresher training will be required for <br /> all supervisors and/or employees who, at any time, by their work practices, fail to show <br /> competence in the areas presented in the initial Hazard Communication training or during work <br /> being performed at the project site. These individuals are considered to pose a potential <br /> hazard to themselves and others in the workplace. <br /> 10.5 Recordkeeping <br /> A copy of all training records shall be kept in the employees' health and safety file. These are <br /> separate records from personnel files and shall have limited access and be kept locked at all <br /> times. Certificates of completion shall be given to any employee who successfully completes a <br /> prescribed training session regardless of the time requirements of the course. The certificate <br /> shall include the following information: <br /> name of participant <br /> name of training course <br /> - name of training provider <br /> -4- company name of participant <br /> date of completion <br /> •4 unique certificate number <br /> >4- expiration date [if applicable] <br /> A copy of all training certificates shall be kept by SB Environmental. Training records must be <br /> updated annually. They shall be retained by each employer for I year, or until the required <br /> updated training has been completed, whereupon that record is kept in the employee's current <br /> health and safety file, replacing the expired certificate. In addition, SBES will keep a record of <br /> training certificates for each individual entering the CRZ and Exclusion Zones. These records <br /> will include the aforementioned training records, respiratory release sign-off and site <br /> orientation signature sheet.
