Information and instructions for obtaining a permanent California EPA ID number (form <br /> enclosed): Complete the application for a California Hazardous Waste Permanent ID Number <br /> and submit it to the DTSC by fax, mail, or email. <br /> 12. Board of Equalization UST Storage Fee Account Number—Call (916) 322-9669 to obtain a <br /> current number for your facility. <br /> 13. San Joaquin Valley Air Pollution Control District—Call (209) 557-6400 to obtain an operating <br /> permit for the UST vapor recovery system. <br /> 14. Food facility operations— Call Leyna Huynh at (209) 468-3446 for operating permit information <br /> for opening the retail food facility. A consultation appointment may be needed. <br /> 15. Go to: htt�://www.sicehd.com/Proarams/Unitiii/unitiii htm for additional information, updates, <br /> regulations and forms. The most current versions of all forms must be submitted. <br /> Also included is a list of our upcoming training classes offered to San Joaquin County businesses for <br /> FREE. Please consider attending one of these free hazardous waste or underground storage tank <br /> classes. <br /> The following is the required follow up and routine testing for your facility: <br /> 1. Annual monitoring system certification, leak detector testing, and spill container testing were <br /> last performed on February 2, 2018. These tests are required once every 12 months. <br /> 2. Secondary containment testing was conducted on June 29, 2016.
