Kennedy/Jenks Consultants <br /> ! and 3 additional monitoring wells During LH's first site investigation, 7 soil borings and 2 <br /> monitoring wells were installed The analytical work performed on the collected soil <br /> samples indicated TPH at a maximum of 12,000 ppm in the soils at a depth of about 15 <br /> feet in the northwest corner of the site All of the drill cuttings from both BCA's and LH's <br /> site work was segregated by soil boring/well and is currently labelled and contained in 55- <br /> gallon drums on the property Figure 1 is a site plan of 102 S Wilson Way <br /> 3 2 Site Activities <br /> Site activities will consist of taking an inventory of all on site drums, collection of soil <br /> samples from the existing soil stockpile or aerated stockpile, periodic groundwater <br /> monitoring and sampling, and supervision of work to backfill the existing excavation with <br /> clean fill material Any work that will include intrusive investigations will require an <br /> activity-specific health and safety addendum to this plan <br /> 4 0 SAFETY AND HEALTH RISK EVALUATION <br /> 4,1 Potential Physical Hazards <br /> Field personnel should be cognizant of potential physical hazards associated with use of <br /> heavy equipment and electrical equipment during field operations Appropriate precautions <br /> include the following <br /> C ANSI-approved hardhats, safety glasses or goggles, and steel-toe boots <br /> will be worn <br /> G Loose clothing that may catch in moving parts will not be worn. <br /> 4 Hearing protection will be worn if a preliminary noise survey or past <br /> experience indicates that maximum noise levels will exceed comfortable <br /> levels at any time during site operations <br /> Additionally, field personnel should not enter any excavations exceeding 5 feet in depth <br /> unless the excavations are properly shored, braced or sloped and a safety ladder is <br /> provided for ready access or egress The existing excavation is approximately 3 feet deep <br /> K/J personnel will not enter any confined space, defined by OSHA as the concurrent <br /> existence of the following conditions, without advanced specific preparation, planning, <br /> training, and supervision by the Regional Safety Supervisor <br /> 1 Existing ventilation is insufficient to remove hazardous air contaminants <br /> and/or an oxygen deficiency exists <br /> 2 Ready access or egress for the removal of a suddenly disabled employee is <br /> difficult due to location and/or size of the opening <br /> 51 mMM&SPIaw 4 910557 00 <br />
