f
130 South Wilson Way December 1,2011
A ARCO Station No. 5469 page 2
be cut off 6 inches above the bottom of this excavation, and removed. The grout used to
.seal the well shalt then be topped off and allowed to spill over into the excavation and fill
it for a thickness of one foot to farm a `mushroom cap' over the top of the cut casing;
If `nested' wells were installed at the site and need to be destroyed by ove'rdriliing, an
auger larger than the original auger must be used and centered on the deepest casing .
(which is off-center in the original.borehole). The auger needs to be sized sufficiently to
reach out and remove all well casings, screens; filter packs, and grout sealant installed
during construction.
All groundwater displaced to the surface during grout installation shall be pumped out of
the excavation and containerized for proper disposal.
• Any wells located offsite require updated access agreements and/or encroachment
permits to be submitted with the, EHD Well Destruction Permit Application.
Deadline for submittal of the work plan is December 30, 2011. After the EHD has approved the
work plan you may submit an EHD Well Destruction Permit Application along with the
appropriate fees. Current fees are: $375.00 per permit, (one permit per parcel) plus $125.00
per each well on the parcel that is to be destroyed. A nested well is considered to be one well.
If you have any questions please contact tori Duncan at (209)468-'0337 or by email at.
Iduncan&sjcehd.com.
Sincerely,
COiX
Lori Duncan, REHS
Senior Registered Environmental Health Specialist
t,
c: James Barton, PG, CVRWQCB, 11020 Sun Center Drive #200, Rancho Cordova, CA 95670
David Evans, Arcadis 950 Glenn Drive#125, Eolsom,``CA 55630
E
t k
S
