The permit has been <br /> approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact John Alaniz at (209) 468-3147 to arrange the following inspection appointments (a 48- <br /> hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Witness correct installation of Bravo Deflector Plates and ensure that any leak from product <br /> piping will be captured by the under dispenser containment unit. <br /> Please be aware that the facility cannot operate the UST system or receive fuel in the <br /> underground storage tank prior to the EHD inspection and approval. <br /> 4. Secondary containment testing for any UST components installed or affected is required six <br /> months after installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 5. If concrete will be broken near the UST or piping, a secondary containment test must be <br /> performed on the [UST and/or piping] once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 6. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> 1868 E.
