(Ground <br /> Zero) requested a 30-day extension to begin work as proposed in Workplan forAddkional <br /> Subsurface Investigation dated October 18, 2000 prepared by Ground Zero for the above- <br /> referenced site. Ground Zero also expressed in the letter that an additional well permit <br /> should not be required by San Joaquin County Public Health Services, Environmental <br /> Health Division (PHS/EHD) for the proposed monitoring well installation since the work is <br /> being performed by the same contractor(Spectrum Exploration) who had previously <br /> performed at the site in February 2000. Ground Zero therefore maintained that the well <br /> permit (#SR0021461) obtained by Spectrum Exploration should still be valid. PHS/EHD <br /> has the following comments: <br /> The request to extend the deadline to begin work is approved. The new deadline to begin <br /> work is January 16,2000. <br /> Please be aware that a well/pump permit is issued to a single G57 licensed contractor for a <br /> single parcel. The permit is valid for a period of one year from the date of issue for the work <br /> proposed and drawn on the permit. Permit#SR0021461 was issued to Spectrum <br /> Exploration from PHS/EHD on December 15, 1999 for the work as proposed in Workplan <br /> for Subsurface Inustigxtion dated October 13, 1999 prepared by Ground Zero. Permit <br /> #SR0021461 is valid up to December 15,2000: Therefore, a new well permit and fee is <br /> required on or after December 15,2000. <br /> A well/pump permit is issued for the specific work drawn (or referred to) on the permit <br /> application.
