Obtain permission to sample and report(in all quarterly monitoring reports)the groundwater sample <br /> from MW-7 owned by the UHaul station located at 2701 North Wilson Way. <br /> 4. Submit well permit application and$89 permit fee to this office. Permit must be obtain before <br /> commencement of work <br /> 5.
