CITY OF LODI <br /> PUBLIC WORKS DEPARTMENT ADDENDUM NO. 1 <br /> PROJECT: UNDERGROUND TANK INSTALLATIONS <br /> 230 West Elm Street and 1331 South Ham Lane <br /> Notice is hereby given that the plans and specifications for the above <br /> mentioned project are being reissued for bidding with the changes listed <br /> below. The plans and specifications consist of the following: <br /> 1. Underground Tank Installation specifications dated May 1988; <br /> 2. Plan sets D44 and D45 issued with the specifications; <br /> 3. This Addendum No. 1 which includes the following changes to the specifica- <br /> tions dated May 1988: <br /> SPECIFICATIONS <br /> Section 1 - Notice Inviting Bids <br /> ° The new bid opening date is Wednesday, July 13, 1988, at 11:00 a.m. <br /> ° C-61 specialty license with proper endorsements for the work is added as an <br /> alternate to the required Class A license. <br /> Section 3 - Proposal <br /> ° The proposal form is replaced with the attached proposal form. Use of this <br /> proposal shall constitute acknowledgement of receipt of Addendum No. 1. <br /> Section 6 - Special Provisions <br /> ° Section 6-05.03, Building Permits - Add the following: <br /> "The City has applied for and received partial approval for this project from <br /> the San Joaquin Local Health District. However, the application will not be <br /> completed and approved until the Contractor is selected. The Contractor <br /> shall provide, to the District' s approval , name and license information, soil <br /> testing laboratory, tank disposal plans, and any other information and <br /> procedures as the District deems necessary to complete and approve the <br /> permit. District fees will be paid by the City." <br /> ° Section 6-150.02, Removal/Abandonment of Existing Tanks and Piping Systems, <br /> Abandonment - Add to 5a: <br /> "The design of the sand and cement slurry mix shall be submitted to the <br /> Engineer for approval a minimum of five days prior to use. " <br /> ° Section 6-150.12, Testing of Piping - Add the following: <br /> "D. The Contractor shall notify the San Joaquin County Air Pollution Control <br /> District upon the completion of work and shall arrange for a blockage <br /> test of all underground vapor return piping. The District and the <br /> Engineer shall witness the test." <br />
