You stated you did receive a fax copy of it,but it was not legible. <br /> You requested a copy of the NOR Subsequently, a copy of the NOR and an explanatory <br /> letter were mailed to you on May 26, 2000. <br /> I <br /> The explanatory letter directed you to submit a wor plan to define the lateral and vertical <br /> extent of the contamination at the dispensers and fo r 8,000 gallon underground storage ' <br /> tank byJuly20, 1999. This work plan has not been received. Anew date for submission of <br /> the work plan to PHS/EHD has been established fc r you. Submit a work plan to <br /> PHS/EHD no laterthan August 15, 2000. Failure t complywith PHS/EHD directives <br /> may jeopardize your eligibility status with the Califo 'a Petroleum Underground Storage <br /> Tank Cleanup Fund. <br /> A proposal for a 2000 feet well survey should be in a initial work plan. Appendic A of dx <br /> Tri Regional Rawrwdatians-Reponing recommends Lhe initial work plan to include providing <br /> information on the proximity of private,municipal, and irrigation wells within 2000 feet of <br /> the site.
