' MONITORING AND REPORTING PROGRAM NO R5-2003-713 <br /> - 2 - <br /> FRANK GUINTA AND GUINTA ENTERPRISES, <br />' JAMES AND MARILYN RAMSEY, <br /> 2072 WEST YOSEMITE AVENUE, MANTECA, SAN JOAQUIN COUNTY <br />' GROUNDWATER MONITORING <br />' The groundwater monitoring program for these wells shall follow the sampling and analytical <br /> program presented in Tables 1 and 2 below Any new wells or domestic well treatment systems <br /> added to this program will initially be sampled on a quarterly basis for the constituents listed in <br />' Table 2 Sample collection and analysis shall follow standard EPA protocol Annual <br /> groundwater monitoring shall occur during the 4th quarter of each year Semi-annual <br /> groundwater monitoring shall occur during the 2nd and 4`h quarters of each year <br />' REPORTING <br />' When reporting data, the Dischargers shall submit the information in tabular form so that the <br /> date, the constituents, and the concentrations are readily discernible The data shall be <br /> summarized in such a manner as to illustrate clearly the compliance with this Order <br />' In addition, the Dischargers shall notify the Board within 24 hours of any unscheduled shutdown <br /> of any remediation system lasting more than 48 hours <br /> As required by the California Business and Professions Code Sections 5735 7835, <br /> and 7835 1, <br /> all reports shall be prepared by a registered professional or their subordinate and signed by the <br /> registered professional <br /> The Dischargers must comply with Article 12 of the Underground Storage Tank Regulations <br /> (Sections 2729 and 2729 1 of California Code of Regulations), which requires Responsible <br /> Parties to submit analytical and site data electronically to Geotracker Attached is our letter <br /> (Required Electronic Deliverable Format for Laboratory and Site Data Submittals to Regulating <br /> Agencies) explaining how to obtain information to implement this requirement <br />' Quarterly Reports and an Annual Report should be prepared as follows <br /> Quarterly Reports Quarterly reports shall be submitted to the Board by the 1st day of the second <br /> month following the end of each calendar quarter (i.e , 1 May, 1 August, and 1 November) <br /> until such time as the Executive Officer determines that the reports are no longer necessary <br /> Each quarterly report shall include the following minimum information <br /> (a) a description and discussion of the groundwater sampling event and results, including how <br /> and when samples were collected, <br /> (b) field logs that contain, at a minimum, wateruaht parameters measured before during <br /> q Y P �, and <br /> after purging, method of purging, depth of water, volume of water purged, etc , <br /> c groundwater{ } gr potentiometric surface maps for all groundwater zones (Water Table, <br /> Intermediate, Deep), <br /> (d) a table showing well construction details such as well number, groundwater zone being <br />
