u <br /> PUBLIC HEALTH SERVICES <br /> p ApUIlY <br /> SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DIVISION Y <br /> Karen Furst, M D , M P H , Health Officer _ P <br /> 304 East Weber Avenue, Third Floor • Stockton, CA 95202 Qe,FORa` <br /> 2091468-3420 <br /> FRANK GUINTAE� � � <br /> GUINTA ENTERPRISES <br /> 305 N UNION ROAD <br /> MANTECA CA 95377 <br /> RE DIRECTIVE TO COMPLETE AN EXPEDITED SITE CODE 2426 <br /> SITE ASSESSMENT <br /> FRANKS EXXON <br /> 2072 W YOSEMITE AVE <br /> MANTECA <br /> San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) provides the <br /> following comments and directives <br /> Due to the vridespread and significant concentrations of contaminants observed at this site, <br /> PHS/EHD directs you to perform a detailed, expedited site assessment using depth discrete and <br /> • continuous core sampling to define and quantify the full three-dimensional extent of suspected <br /> contarrunants in soil and groundwater Analytical data from the site listed above confirms the presence <br /> of petroleum hydrocarbons in groundwater and drinking water from wells north of the subject site A large <br /> mass of MTBE and other fuel hydrocarbons are present which may endanger beneficial use groundwater <br /> This directive is issued considering several factors and is supported by the Central Valley Regional Water <br /> Quality Control Board(CVRWCQB) In addition you are directed to continue to provide a reliable potable <br /> water supply to each affected residence until further notice <br /> Please evaluate the potential for additional underground storage tank related sources to exist to the north of <br /> the subject site reported to PHS/EHD(both parcels north-west and north-east across W Yosemite Avenue) <br /> Report your findings to PHS/EHD <br /> Assessment of affected private domestic and irrigation .}ells shall be completed In the absence of well <br /> construction details a down-hole video log shall be completed Removal of the pump/conveyance system <br /> and interruption of water supply to each residence for an undetermined duration is likely Prior to down- <br /> hole logging please clear the internal well casing walls (brush) and remove debris (sediment) to maximize <br /> data collection Each well affected by contaminants associated with an unautnonzed release shall be <br /> assessed Please document total depth, depth of pump, discharge capacity, inside diameter, casing material, <br /> casing condition, screened mtervaI(s), contaminants and concentrations observed, and distance from <br /> contamination sources Site each well on cross sections requested below(to scale) <br /> Plume definition vertically and laterally shall be completed. Please note, you are requested to <br /> immediately pursue any off-site access agreements that you may need to complete your investigation <br /> activities. The groundwater plume may include more than 12 square acres in plot view PHS/EHD <br /> strongly recommends the use of continuous core/continuous logging technologies The total depth of <br /> assessment is not prescribed but should consider the total depth of impacted water wells and significant <br /> . pumping influences regionally Report your results in the Soil and Water Investigation (SWI) Report <br /> requested below <br /> You are requested to properly characterize the hydrogeology and groundwater flow conditions in the <br /> vicinity of your site. During SWI activities, detailed stratigraphic information must be gathered using <br /> boxings, continuous core sampling, dual tube expedited site assessment methods, or cone penetrometry <br /> together with other methods to #16M(311@ Services <br />
