SAN" J D�l+ �� I 1411" Environmental Health Department <br /> COUNTY�l <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Big Lots Manteca 4617 1321 W YOSEMITE AVE Manteca July 16 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> The following observations were made in the hazardous waste storage area of the facility(IMG 7383-4): <br /> 1.A Compressed Gas& Flammable(CG& FL) HW tote contained the following liquid waste items, but the HW tote <br /> label was marked with only"solid"for the physical state(IMG 7386-91): <br /> a. Five bottles of nail polish <br /> b.A 1 fl oz container of Revlon Photoready Airbrush Effect makeup foundation <br /> Samira Hamed,Assistant Store Manager, marked liquid as the physical state on the CG& FL HW label (IMG 7448). <br /> 2.A bag containing used absorbent, and labeled "Lysol All Purpose Cleaner"and with UPC#019200752176,was <br /> found in the Corrosive Bases(CB) HW tote. However,when Samira entered the UPC number into the hand held <br /> scanner, the scanner stated the Lysol cleaner is classified as TX. Samira stated the TX meant that the bag of used <br /> absorbent should have been placed in the Toxic&State Regulated (ST) HW tote. Samira placed the bag of used <br /> absorbent into the TX&ST HW tote. (IMG 7397-403) <br /> 3.A bag containing used absorbent, and labeled "Snuggle"and with UPC#077180111,was found in the CB HW tote. <br /> The Non-Saleable Hazardous Material Checklist document, also adhered to the tote, stated the Snuggle was a <br /> detergent, although Snuggle is known to be fabric softener(a laundry additive).When Samira entered the Snuggle <br /> UPC number into the hand held scanner,the scanner stated"Product Not Found."According to the Big Lot's Waste <br /> Disposal Quick Reference Guide posted on a wall next to the HW storage area, soaps and detergents are classified <br /> as Toxic HW and"some laundry additives"are classified as Corrosive Acid HW. Samira was not sure why the used <br /> absorbent containing Snuggle was placed in the CB HW tote. (IMG 7404-5, 7408-10) <br /> 4.A partially filled 1.01 fl oz container of Dead Sea Collection Cocoa Butter Facial Serum was found in the Oxidizer <br /> (OX) HW tote. However, the HW label on the tote was marked as only"solid"for the physical state.Also,when <br /> Samira scanned the facial serum,the scanner stated "Product Not Found."Samira could not explain how it was <br /> determined that the facial serum should be placed in the OX HW tote. (IMG 7411-20) <br /> 5. The Toxic&State Regulated (TX&ST)HW tote contained the following liquid waste items, but the HW tote label <br /> was marked with only"solid"as the physical state(IMG 7421-26, 7429, 7434-9, 7445) <br /> a. Two 1.7 fl oz containers of Pantene Pro-V hair conditioner, which were missing caps. <br /> b.A 12.6 fl oz container of Pantene Pro-V Hydra Scalp Dandruff Shampoo <br /> c.A 15 fl oz container of Oil of Life Body Wash <br /> d.A 4 fl oz leaking container of Dead Sea Collection Collagen Body Oil <br /> e.A leaking container of Children's Benadryl <br /> f.A Covergirl mascara <br /> g.A Covergirl make-up concealer <br /> h.A Rimmel mascara <br /> Samira marked liquid as the physical state on the TX&ST HW label (IMG 7447). <br /> 6. The aerosol cans listed below were found in the TX&ST HW tote.When Samira scanned the aerosol cans,the <br /> scanner stated the aerosol cans are classified as Compressed Gas(CG).
