S ■ N*JOA N I N Environmental Health Department <br /> COUNTY— <br /> Small <br /> OUNTYSmall Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Big Lots Tracy 4457 2681 N Tracy Blvd, Tracy June 25 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> -An Oxidizer(OX) Hazardous Waste(HW)tote, containing a bottle of Murphy Oil Soap,was marked with two different <br /> accumulation/collection start dates, 2/4/19 and 5/1/19. Marely Martinez, Assistant Store Manager, crossed out the <br /> 2/4/19 and replaced it with the date 5/1/19.The most recent hazardous waste manifest was dated 4/29/19. (IMG <br /> 7056-62) <br /> -The following was found in a Toxic&State(TX&ST)HW tote,with the physical state marked as solid: <br /> a.A seemingly empty 40 oz container of Gain detergent,without used absorbent or spill clean-up material to <br /> account for the missing liquid. Marely did not know what happened to the missing liquid. (IMG 7063-7) <br /> b.Two partially empty 32 oz containers of Ortho Weed B Gon weed killer liquid,without used absorbent or spill <br /> clean-up material to account for the missing liquid. One of the containers was leaking out of the bag it was contained <br /> in and into the tote. Marely did not know what happened to the missing liquid. Marley cleaned up the leakage and <br /> placed the spill clean-up material into a new bag with the leaking container.The active ingredients listed on the weed <br /> killer label were dimethylamine salt of 2,4-D acid,dimethylamine salt of mecoprop-p acid, and dicamba acid,while the <br /> inactive ingredients were listed as 2,4-D acid, mecoprop-p acid, and dicamba acid. Marely did not know why the weed <br /> killer was placed in the TX&ST HW tote, rather than the Corrosive Acid(CA) HW tote. Marely placed the weed killer <br /> containers into the CA HW tote. (IMG 7074-9, 7090-1) <br /> c.A partially empty leaky 16 fl. oz. container of Spa Haus Teatree Mint Body Wash liquid,without used absorbent <br /> or spill clean-up material to account for the missing liquid. Marely did not know what happened to the missing liquid. <br /> (IMG 7084-5) <br /> Marely did not mark the liquid physical state on the TX&ST HW tote label at the time of inspection. <br /> - A Corrosive Base(CB)HW tote,with the physical state marked as solid, contained two non-empty containers of <br /> Dynamo Free&Clear liquid laundry detergent, one was 60 oz. in size and the other was 100 oz. in size.According to <br /> the TX&ST HW tote label and the Big Lots Waste Disposal Quick Reference Guide(Guide), soap and detergent are <br /> classified as TX&ST HW. (IMG 7068-74) <br /> Marely did not mark the liquid physical state on the CB HW tote label at the time of inspection. <br /> -An open blue tote was observed enclosed in an unlabeled plastic bag and on the ground adjacent to the HW storage <br /> area. Marely did not know what was in the tote. Marely opened the bag and revealed the following waste contained in <br /> the tote(IMG 7092-7098): <br /> a.Approximately 11 cans of La Croix sparkling water coated in an unknown white powdery substance. This white <br /> substance was also observed on the inside walls of the tote. <br /> b. A partially empty and open 6 fl oz container of Cutter All Family Insect Repellent. Marely did not know what <br /> happened to the missing liquid.The insect repellent was labeled as combustible, and according to the repellent's <br /> Safety Data Sheet(SDS),the repellent is flammable. <br /> c. A seemingly empty and open 44 fl oz container of Ultra Downy Odor Protect Fabric Conditioner+Deodorizer. <br /> Marely did not know what happened to the missing liquid. <br /> d.A partially empty and open 18 fl oz container of Dawn Ultra Dishwashing Liquid.The liquid that remained in the <br /> container was orange in color.This rest of this orange liquid from the container was observed spilled on the bottom <br /> FA0003106 PRO538909 SC001 06/25/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 5 of 11 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
