The facility's business plan was not submitted electronically to the California Environmental Reporting System (CERS)for 2019 and 2020. As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 4 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Activities and Business Owner/Operator Identification forms have not been submitted for 2019 and 2020. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class II. 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. At least 1,200 pounds of carbon dioxide and 1,368 cubic feet of compressed nitrogen were found on site and have not been reported for 2018 and 2019. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at http-.Hcers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. This is a repeat violation, Class II. 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. A site map was not completed or submitted as part of the business plan for 2019 and 2020 and the last map submittal does not show/address the following elements: 1. Multiple maps are not numbered (i.e., map 1, map 2, map 3, and map 4). 2. North orientation. 3. Loading areas. 4. Internal roads (none). 5. Storm and sewer drains. 6. Evacuation staging area. 7. Access and exit points are not identified (from the street). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
