Nitrogen gas(north Stadium Club)—the amount is wrongly reported in pounds vs. cubic feet. 2. Carbon Dioxide (north Stadium Club)—the storage temperature is wrongly identified as "above ambient"vs. "ambient". 3. Carbon Dioxide (Soccer Field House) -the storage temperature is wrongly identified as "above ambient"vs. "ambient". 4. Nitrogen gas(South Stadium Club)—the amount is wrongly reported in pounds vs. cubic feet. 5. Carbon Dioxide (South Stadium Club) -the storage temperature is wrongly identified as "above ambient"vs. "ambient". • Violation#6,the site map does not show or address the following: 1. Seven maps submitted, only six are numbered. 2. Loading areas have not been addressed. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". • Violation#7, Emergency Response/Contingency Plan is not complete. 1. Section E. Facility Evacuation; box E3,the Emergency Assembly Area has not been identified and the check boxes in box E4 have not been completed. • Violation#11—Records showing the employees have received training in the following elements have not been received (see attached sample employee training record). 1. Familiarity with the emergency response plans and procedures in the event of a release or threatened release of a hazardous material; (Emergency Response/Contingency Plan familiarization). 2. Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency; (spill reporting).
