Please clarify and complete the section <br /> under UST Operator from 3/15/88-10/30/02 on page three. <br /> • On page two(2) of your claim application, complete the section for the Date UST Removed <br /> (12/9/98?). <br /> • Additionally on page two,you list the Date Release Discovered as 12/9/98 (this date is also listed on <br /> the Underground Storage Tank Unauthorized Release(LEAK)/Contamination Site Report) and the <br /> Date Regulatory Agency Confirmed Release and Issued Cleanup Directive to be on 4/19/99;however, <br /> Geo Tracker lists the Leak Discovery date as 5/13/92. Please clarify and provide verification from <br /> the local agency that an unauthorized release of petroleum from the UST was discovered on a <br /> specified date. <br /> • Provide a copy of the first letter from the local regulatory agency naming you a responsible party and <br /> directing you to cleanup the contamination at the subject site. The earliest dated letter you provided <br /> (from San Joaquin County EHD) is dated 8/10/01 and is for three different sites. If 4/19/99 is the <br /> correct date (as listed on page two of your claim application),provide written verification from the <br /> Regulatory Agency to confirm this. <br /> • Page four(4)of your claim applications indicates there is an insurance policy with American <br /> International Specialty Lines (Policy No.PLS6191639). Refer to pages 18-19 of the enclosed <br /> Financial Responsibility (FR) Guide for reference to using Insurance as a Financial Responsibility <br /> mechanism and provide a copy ofyourinsurance policy. <br /> • Page six (6) of your claim application indicates you are using Selflnsurance as the mechanism used <br /> for demonstration of Financial Responsibility. For Self Insurance you must have a tangible net worth <br /> of at least$10 million and must complete the Chief Financial Officer Letter(CFO).
