S0 JOA U 1 Environmental Health Department <br /> U ; i y <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Walgreens#15398 660 W March Ln, Stockton July 16, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br /> were: <br /> 1.A container storing ignitable hazardous waste was not labeled with the generator's address. Scott Barnett,the <br /> store manager,wrote the generator's address on the container's label at the time of inspection. <br /> 2. Two containers, one located in the hazardous waste storage area and another located in the photo department, <br /> were observed storing used photo ribbon rolls.According to Scott, up until about one year ago,the facility would <br /> throw the used photo ribbon rolls into the trash, but now Stericycle,the facility's hazardous waste hauler, takes the <br /> rolls during their hazardous waste pick-ups. The containers of used photo ribbon rolls were not labeled with the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), and accumulation start date. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br /> action statement and supporting documentation to the San Joaquin County Environmental Health Department <br /> (EHD)within 30 days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on 7/27/2020 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 8/26/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
