Monitoring records include: (1)date and time of <br /> all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual <br /> observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br /> site and submit copies to the EHD. <br /> This is a Class II violation. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> The designated operator's inspection reports and the manager onsite indicated that UDC 3/4 was impacted by a <br /> truck in September of 2018 and they are waiting to hear from the contractor to replace the dispenser.A repair <br /> permit application was not submitted to the EHD for this service.All repairs must have a permit submitted to and <br /> approved by the EHD prior to beginning work. Immediately, have a properly licensed, trained and certified contractor <br /> repair or replace the damaged component under permit and inspection of EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e)Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> A designated operator inspection was performed on December 20, 2018 and the next inspection was performed on <br /> January 21, 2019, 33 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> The DO failed to check and note all required items on the visual inspection reports. <br /> The following was observed on the designated operator's monthly inspection reports: <br /> -CERS ID was not included. <br /> -The"N/NA"in sections VII through XI were not explained. <br /> -Section VI of the report was the owner/operator's signature. <br /> -The question regarding UDC sensor locations was left blank. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report.
