SAN" J Dl+ I 1411" Environmental Health Department
COUNTYl
Small Quantity Hazardous Waste Generator Inspection Report
Facility Name: Facility Address: Date:
Dollar Tree#04403 1 125 Lakewood Mall Lodi May 10 2019
SUMMARY OF VIOLATIONS
(CLASS I,CLASS II,or MINOR-Notice to Comply)
Item# Remarks
403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous
waste.
-An empty 32 fl. oz. container of hydrogen peroxide, in a HW bag, marked STOU, and with a date of 5/8/2019 was
found on a shelf next to the Dollar Tree baskets containing"go-back"items, located at the front of the facility. David
said he had not had a chance to place the item into a HW tote in the back storage area. When David scanned the
barcode on the hydrogen peroxide, the hand-held scanner stated the hydrogen peroxide should be placed in a
State-Regulated, Toxics, OTC Pharmaceuticals, Universal Waste(STOU)HW tote. However, according to the
Products To Be Treated As Hazardous Waste guide posted next to the HW storage area, hydrogen peroxide is to be
placed in an Oxidizers(OX) HW tote. David placed the hydrogen peroxide into an OX HW tote. (IMG 6152-3, 6223-5,
6289)
-The items listed below were found in a Dollar Tree basket labeled"Resell Damages,"located on the top shelf above
the Dollar Tree"go-back" item baskets. When asked about the items listed below individually, David did not know who
placed the items in the basket, nor why.According to David, items placed in the"Resell Damages"basket are
processed daily(IMG 6154):
o A HW bag containing two broken deodorant sticks. David said the deodorant could not be resold and should be
processed as HW. David scanned an Arm &Hammer Ultra Max deodorant and a Beauty&Skincare Power Stick
deodorant, both of which were indicated by the handheld scanner to be placed in a STOU HW tote. David marked the
broken deodorant sticks with STOU and today's date. (IMG 6155, 6188-91)
o A HW bag containing what appeared to be paper"Bee Yourself'cards. David marked the cards with STOU and
today's date. (IMG 6155-6, 6191)
-Seven HW bags containing various brands of perfume bottles missing the actuators were found in an open cardboard
box on the ground next to the Dollar Tree"go-back"item baskets.A bag of Premium Cat Litter was on top of the bags
of perfumes. Six of the bags contained approximately 13 perfume bottles each, and one bag contained four perfume
bottles.Approximately 82 perfume bottles total, most of which were missing actuators, were found in HW bags. David
did not know how long the perfumes had been in the location they were found. David stated he would have to remove
all of the perfumes from the facility's inventory through the facility's computer, since the perfumes did not have
barcodes to scan. David took the perfumes to the HW storage area, but did not place the perfumes into a HW tote at
the time of inspection. (IMG 6174-8)
-A HW bag containing three 0.6 fl. oz. containers of Grad (blowing)Bubbles was found in the open cardboard box
beneath the bags of perfume. David did not know how long the bubbles had been in the location they were found.
According to the Guide, blowing bubbles are classified as STOU HW. David marked the bubbles with STOU and
placed them in an STOU HW tote. (IMG 6179, 6218-20)
-The list of items below were found on a sales rack labeled "Reduced Merchandise" (IMG 6309-13).According to
David, items that are damaged, but are still saleable, are marked down in price and placed on this rack.
o A Sure Scents air freshener filled aerosol can, missing the actuator, and on sale for 25 cents. Unable to find the
actuator for the aerosol can, David removed the aerosol can from the sale rack and stated he would process the
aerosol can as HW.
All hazardous waste shall be in a closed at all times and all hazardous waste containers shall remain closed except
when adding or removing waste.
FA0020328 PRO539176 SCO01 05/10/2019
EHD 22-01 Rev.09/27/2018 Page 11 of 15 Small Quantity Hazardous Waste Generator OIR
1868 E.
