Ham Ln., Lodi, CA, although the facility's generator address on file with DTSC and listed on the facility's hazardous <br /> waste manifests is 125 Lakewood Mall, Lodi, CA: <br /> o STOU HW tote with accumulation start date 1/23/19 and containing an empty 96 oz container of bleach (IMG <br /> 6194-9). <br /> o Three STOU HW totes with accumulation start dates 1/20/19 and containing various HW items.These totes were <br /> stacked on top of each other in the HW storage area (IMG 6226-88). <br /> -The items listed below were found in a Dollar Tree basket labeled"Resell Damages,"located on the top shelf above <br /> the Dollar Tree"go-back" item baskets.When asked about the items listed below individually, David did not know who <br /> placed the items in the basket, nor why.According to David, items placed in the"Resell Damages"basket are <br /> processed daily(IMG 6154): <br /> o A HW bag containing two broken deodorant sticks. David said the deodorant could not be resold and should be <br /> processed as HW. David scanned an Arm &Hammer Ultra Max deodorant and a Beauty&Skincare Power Stick <br /> deodorant, both of which were indicated by the handheld scanner to be placed in a STOU HW tote. David marked the <br /> broken deodorant sticks with STOU and today's date. (IMG 6155, 6188-91) <br /> o A HW bag containing what appeared to be paper"Bee Yourself'cards. David marked the cards with STOU and <br /> today's date. (IMG 6155-6, 6191) <br /> o A partially empty 4 fl. oz. container of Assured Regular Strength Stomach Relief Bismuth subsalicylate 525 mg. <br /> The seal on the container was broken. David said the medicine could not be resold and should be processed as HW. <br /> David placed the medicine in a HW bag and marked it with STOU and today's date. (IMG 6157-9, 6191) <br /> o A leaky partially empty 6.5 oz. container of Angel Of Mine Baby Oil Gel. David said the gel could not be resold <br /> and should be processed as HW.The handheld scanner stated the oil gel is not hazardous. However, according to the <br /> Products To Be Treated As Hazardous Waste guide(Guide)on site, Baby Oil is classified as STOU HW. David <br /> placed the oil gel into a HW bag and marked it with STOU and today's date. (IMG 6160-1, 6184, 6191) <br /> o A.25 oz. container of Lip Medex Blistex External Analgesic/Lip Protectant,which appeared to be used. David <br /> said the lip protectant could not be resold and should be processed as HW.The handheld scanner stated the lip <br /> protectant should be placed in a STOU HW tote. David placed the lip protectant into a HW and marked it with STOU <br /> and today's date. (IMG 6181, 6191) <br /> o A container of Nature's Measure Energy Dietary Supplement 21 tablets. The seal on the container was broken. <br /> David said the tablets could not be resold and should be processed as HW.The handheld scanner stated"See guide" <br /> for the disposal code for the dietary supplement. Based on the Guide classifying "Energy Bars and Drinks(with <br /> supplemental facts only)"as STOU HW, David placed the supplement tablets into a HW bag and marked it with STOU <br /> and today's date. (IMG 6164-7, 6183, 6191) <br /> o An open 1.5 oz. container of dried modeling dough. David said the dough could not be resold and should be <br /> processed as HW.The handheld scanner stated"See guide"for the disposal code for the modeling dough. Based on <br /> the Guide classifying"Play Dough"as STOU HW, David placed the modeling dough into a HW bag and marked it with <br /> STOU and today's date. (IMG 6168-9, 6182, 6191) <br /> David took all of the above mentioned items to the HW storage area and placed them into a tote and labeled the tote <br /> with a STOU HW label (IMG 6214-5). However, David did not mark the physical state until the inspector pointed out <br /> that the physical state was not marked (IMG 6222). David also wrote the store(generator)address incorrectly as 125 <br /> N. Ham Ln. Lodi, CA on the HW label. <br /> -An empty damaged 96 oz. container of The Home Store Bleach was found in an STOU HW tote marked with <br /> FA0020328 PRO539176 SCO01 05/10/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 13 of 15 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
