Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) handling or the training provided has been inadequate. 1. The following items were observed in a State-Regulated,Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU) HW tote with accumulation start date 1/2/19(IMG 6508-17, 6522-3): a.A 16.9 fl oz bottle of Spa Soap hand soap was found about half empty,without used absorbent or spill clean-up material to account for the missing liquid. Jessica Slefo, Store Manager, did not know what happened to the missing liquid.According to Jessica, items that have been used by employees to clean the store or while at work,will be marked with an orange sticker or labeled with the words"store use." b.A 6 oz container of Sure Scents Air Freshener Crystal Beads was found about half empty,without used absorbent or spill clean-up material to account for the missing beads. Jessica did not know what happened to the missing beads. c.A seemingly empty 33.6 oz bottle of Luminessence Fresh Linen Liquid Potpourri was found with used absorbent. However,the amount of used absorbent did not account for the amount of liquid that was missing from the container.Also, according to Jessica, empty containers that once held hazardous materials are managed as HW. d.A seemingly empty 64 oz container of LA's Totally Awesome Laundry Detergent without used absorbent or spill clean-up material to account for the missing liquid. Jessica did not know what happened to the missing liquid. e.A seemingly empty 64 oz container of LA's Totally Awesome Fabric Softener without used absorbent or spill clean-up material to account for the missing liquid. Jessica did not know what happened to the missing liquid. 2. The following items were found in an STOU HW tote with accumulation start date 2/5/19(IMG 6524-8): a.A partially empty 15 fl oz bottle of Oil of Life Moisturizing Body Wash,without used absorbent or spill clean-up material to account for the missing liquid. Jessica did not know what happened to the missing liquid. b.A seemingly empty 16 oz bottle of Ajax dish soap,without used absorbent or spill clean-up material to account for the missing liquid. Jessica did not know what happened to the missing liquid. 3. The following items were found in a Corrosive Bases(CB) HW tote with accumulation start date 2/15/19(IMG 6529, 6538-6544): a.A single-bag containing saturated used absorbent from a leaking container of LA's Totally Awesome All Purpose Concentrated Cleaner was found leaking out of the bag and into the CB HW tote. Jessica double bagged the leaking item, cleaned up the leak inside the tote using paper towels, and placed the used paper towels into the double bag with the item. 4.The following item was found in an Oxidizers, Reactives(OX) HW tote with accumulation start date 3/19/19(IMG 6545-6547): a.A seemingly empty 24 fl oz bottle of Comet Classic Foaming Bleach Bathroom Cleaner without used absorbent or spill clean-up material to account for the missing liquid. Jessica did not know what happened to the missing liquid. 5.The following item was found in an Aerosols, Flammables, &Combustibles(AF) HW tote with accumulation start date 4/12/19(IMG 6550-6553): a.A broken partially empty nail polish bottle without used absorbent or spill clean-up material to account for the missing liquid.
