Airport Way, Stockton CA Program: TP <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Onsite Tiered Permitting Hazardous Waste Routine Inspection Report <br /> This facility has a Conditional Authorization (CA) to reduce chromium VI to chromium III in unit number <br /> 4-0 "Chromium Treatment System". The monthly treatment volume is 3000 gallons and the chromium <br /> concentration of each batch treated is unknown. Make a determination of the concentration by taking <br /> representative samples at the point of generation. Provide copies of sampling locations and the <br /> analytical test results. <br /> Resin treatment units 1-1 and 1-2 were never documented as being closed. These units were never <br /> used according to Mr. Kitagawa. Submit closure documentation for these units. <br /> A facility inspection was conducted and checklist inspection report was issued on July 11, 2011. <br /> #3. Container management standards - {California Health and Safety Code [HSC], <br /> section 25200.3 (c)(4)). CA Code of Regulations, Title 22 (CCR), section 66264.174. <br /> Hazardous waste containers are not being inspected weekly. At least weekly, the owner or operator <br /> shall inspect areas used for container storage or transfer. An employee was asked how often the unit <br /> area was inspected and was not able to provide the correct answer. Ensure that all hazardous waste <br /> containers are inspected at least weekly and provide adequate training to the employees conducting the <br /> inspections on what is required to be inspected. Class II violation. <br /> #10. Annual waste minimization certification - HSC, section 25202.9 <br /> An annual waste minimization certification was not submitted to the Certified Unified Program Agency <br /> (CUPA) annually. Submit the certification annually using the Unified Program Consolidated Form <br /> (UPCF), Onsite Hazardous Waste Treatment Notification - Facility Page. Class II violation. <br /> #33. Financial Assurance - CCR, section 67450.13 <br /> At the time of the inspection an adjusted estimate of the cost of closing the CA unit was not submitted to <br /> the CUPA since 2007. The estimate must be submitted along with the UPCF Certification of Financial <br /> Assurance to the CUPA by March 1 of each year. During the inspection a adjusted estimate was <br /> provided dated March 8, 2011. Provide a copy of the written estimate of closure costs, the UPCF <br /> Certification of Financial Assurance form and a statement that this facility has sufficient financial <br /> resources to meet the closure cost requirements to the CUPA annually. If the closure cost exceeds <br /> $10,000 a financial assurance mechanism will be required. Class II violation. <br /> Submit a return to compliance certification, with a corrective actions statement outlining each violation <br /> and supporting documentation for each violation noted in the summary of violations by August 13, 2011 <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> j <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
